I, the undersigned, acknowledge and agree that my participation in yogababy courses will be on the following conditions:
· I have informed the teacher of all injuries and illnesses I have or have had, and all medication I am taking or have taken, and any other conditions which may affect my participation in classes;
· I will inform the teacher if my circumstances change and I am suffering from any injury or illness, begin taking any medication, or any other conditions which may affect my participation in the course;
· I will follow the teacher’s instructions during the course and that failure to do so, or to follow these conditions, may expose me to risk of injury;
I am aware the classes will require physical exertion.
